How to make an authoization letter about getting money from an insurance company? please help me.. thanks?

my father asked me to write an authorization letter regarding his claims in an insurance company. im not sure if its benefits or whatever. and im having a hard time thinking of what reason im gonna say so please help me. anyways the insurance company is insular life. thank you very much. just needed it badly
